Consent to the processing of personal data – sending commercial communications

  1. You hereby give your consent to DESK – FORM a.s., with its registered office at Osada Dukla 238, 415 01 Újezdeček, Company ID No. 25467018, entered in the Commercial Register kept by the Regional Court in Ústí nad Labem, Section B, Insert 1481, to process your following personal data pursuant to Act No. 101/2000 Coll., on the Protection of Personal Data:
    a. First name and surname;
    b. E-mail address;
    c. Telephone contact;
    d. Company name.

  2. The processing of your name, e-mail address and telephone contact, and possibly your company name, is carried out for the purpose of including you in a database for sending commercial communications by electronic means, the content of which consists of current information in the field of kitchen worktops—news, inspirational ideas and offers of our products.

  3. You grant consent for a period of 3 calendar years unless you extend this period.

  4. The processing of personal data is carried out by DESK – FORM a.s. as the controller; personal data may also be processed by the following processors:
    a. Provider of software for sending bulk e-mail messages;
    b. Provider of Microsoft Office software;
    c. Possibly other providers of processing software, services and applications, which the company does not currently use.

  5. The consent to the processing of personal data may be withdrawn at any time by any means. Withdrawal of consent will result in deletion of the given personal data from the database intended for sending commercial communications.

  6. Please note that under the Personal Data Protection Act you have the right to:
    • request information about which of your personal data we process,
    • request an explanation regarding the processing of personal data,
    • request access to such data and have it updated or corrected,
    • request deletion of such personal data,
    • in case of doubts regarding compliance with obligations related to the processing of personal data, contact the company or the Office for Personal Data Protection (Úřad pro ochranu osobních údajů).
